What causes painful ejaculation?

Sexual medicine research indicates that painful ejaculation is mostly caused by organic lesions of the reproductive organs. The main causes include:

Urinary tract and sexual organ infections: Lower urinary tract infections in men are the most common cause of painful ejaculation, including urethritis, cystitis, and prostatitis. These infections can cause local tissue congestion and swelling, lowering the pain threshold. The series of rhythmic contractions during ejaculation provides mechanical stimulation to the inflammatory lesions, often accompanied by severe pain during ejaculation. Additionally, seminal vesiculitis and epididymitis can also cause painful ejaculation. This is because, in addition to the pain caused by the lesions themselves, the discharge of secretions and sperm into the vas deferens further exacerbates the pain. Some patients may experience painful nocturnal emissions.

Malignant tumors: Malignant tumors of the male urogenital organs often cause painful ejaculation. For example, when tumors occur in the testes or epididymis, the pressure generated during sexual activity, as well as the rotation and elevation of the testes, can cause pain. However, the pain is usually milder, often described as a dull ache or aching. While tumors in the prostate, seminal vesicles, and vas deferens are rare, their progression is often insidious, making this type of pain a warning sign.

Urinary tract stones are common in men; bladder stones and ureteral stones in the lower urinary tract can both cause painful ejaculation. This is because stones are hard foreign bodies, and during ejaculation, the contraction and peristalsis of local soft tissues compress the stone, causing pain. Severe urinary tract stones can lead to secondary infections, further exacerbating the pain.

In addition, conditions such as tuberculosis of the male urogenital system, urethral polyps, and Peyronie's disease can also cause painful ejaculation; overly vigorous intercourse or excessive sexual activity that overworks the gonads can also cause pain.
