Is SM a sexual perversion or a human rights violation?

Media reports have indicated that the arrest of the owner and performers of a shop providing live SM performances
sparked protests by the "Rainbow Action" group, who accused the police of suppressing a minority group. Such reports have
further fueled public debate about whether SM is a basic personal "sexual right" or a form of "sexual perversion" requiring
treatment.
This article aims to discuss SM-related issues from a psychiatric perspective.
SM is an acronym derived from the first letters of the English terms sadism and masochism
, generally referring to sadism and masochism in sexual behavior.
Sexual sadism refers to the condition where a person derives sexual pleasure
only by inflicting psychological or physical pain (including humiliation) on their partner or victim through sexual fantasies, impulses, or actions
; conversely, sexual masochism refers to the condition where a person derives sexual pleasure only by inflicting psychological or physical
pain
.
Some people suffer from both sexual sadism and sexual masochism simultaneously
, finding pleasure in causing pain to others and in enduring pain inflicted upon them.
However, occasional "rough" behavior during sexual intercourse between couples may be normal.
According to foreign surveys, approximately 5% to 10% of couples in the United States have engaged in
actions such as scratching, hitting, pinching, or biting during sexual intercourse and derive sexual pleasure from them.
In these cases, both partners are usually quite rational and able to maintain boundaries and respect each other,
unlike those with sadomasochism or masochism, thus avoiding real harm to one another.
Sadomasochism is named after Marquis des
des des (1774–1814), an 18th-century French writer and military officer who was repeatedly
imprisoned for his violent sexual acts against women.
Masochism, on the other hand,
is named after Lepold von Sach er-masoch (1836–1905), a 19th-century Austrian novelist who, along with
his characters, derives sexual pleasure from the abuse and control of women.
According to the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition
(DSM-4), published by the American Psychiatric Association in 1994, sadomasochism and masochism are classified as paraphilias, but with strict diagnostic
criteria: such behavior must be recurrent and persistent for more than six months, and clinically cause
significant impairment in social, occupational, or other important functions.
Sadomasochism is often associated with rape, and some individuals even kill their partners after sexual intercourse, becoming known as
"least murderers." Many of these individuals have latent schizophrenia
; some scholars believe they have dissociative identity disorder and may also have a history of brain injury.
The etiology is currently unclear, but it may be related to genetic predisposition, hormonal imbalances, pathological
interpersonal relationships , a history of sexual abuse, and other mental illnesses. Like sadomasochism, masochistic behavior is more common in men.
Male masochism is cyclical, involving a deliberate seeking of masochistic situations to achieve sexual arousal and orgasm;
while female masochism is not cyclical but continuous, only manifesting when life becomes unbearable. Therefore, female masochism is considered a lifestyle, and those with sexual masochism may have experienced and believed in pain as a prerequisite for sexual pleasure
in childhood . Psychoanalysts consider sexual masochism to be self-inflicted sadism; 30% of those with sexual masochism also have sadistic fantasies, while moral masochism does not include sexual fantasies. Currently, there are no effective clinical treatments for sexual masochism and sexual sadomasochism; sexual masochism often involves criminal law, and the potential harm to sexual partners or victims should not be underestimated. In conclusion, from a psychiatric perspective, SM (sexual masochism and sexual sadomasochism) is a disorder of sexual fetishes that requires clinical treatment; it can cause serious physical and psychological harm to individuals or others, and may even involve criminal offenses such as assault or murder. Therefore, while the manner of sexual behavior is a personal matter and should be protected and respected, if it has reached a pathological level (such as sadomasochism or masochism), further evaluation, treatment , and follow-up by professionals are necessary to minimize potential harm. The word "sadism" originates from an 18th-century French erotic writer, the Marquis de Sade ( original name Donatien Phonsef Rancois, de Sade). Because he extensively discussed sadomasochistic themes in his works, he is considered the founder (or at least , and thus his name is used as a synonym for this kind of behavior. As a side note, the film "Quills" (not "quill," as "quill" would be "Korru"!) is based on unofficial histories of the Marquis de Sade, but the film differs greatly from historical accounts, so watch it for entertainment purposes and don't treat it as historical fact. In addition, Crown Publishing recently translated and published one of the Marquis de Sade's most controversial works, " The 120 Days of Sodom." The term "masochism" originates from the 19th-century Austrian writer and journalist Leo Poldrittervonsacher-Masoch, who wrote a novel called *Venus in a Beast Skin* ( or *Venusimpelz* in German), which contains numerous descriptions of the psychology of the masochist . This description also reflects his own life experiences, thus leading to its association with sadism as a form of psychological manipulation.
