Infertility caused by abnormal sexual intercourse

Inability to have normal sexual intercourse can also cause infertility. Inability to have normal sexual intercourse refers to the male partner experiencing difficulty or being unable to penetrate the vagina. One cause is vaginismus, a psychophysiological syndrome caused by factors such as female anxiety. It is due to involuntary spasms of the muscles surrounding the vaginal opening and the outer third of the vagina. This may be a protective conditioned reflex caused by injury or pain to the vulva or vaginal opening, but it is more often caused by non-organic factors. Treatment should include psychological and physiological adjustments, along with appropriate psychotherapy techniques. Organic causes of intercourse difficulties should be effectively treated, such as eliminating inflammation. Other causes of penile penetration include various congenital abnormalities, such as congenital absence of the vagina or incomplete vaginal development (only 1-4 cm long), vaginal atresia or stenosis, and imperforate or rigid hymens or vaginal diaphragms. In men, conditions such as hypospadias, large inguinal hernia, micropenis, double penis, and penile curvature can cause infertility. Other conditions include penile-scrotal elephantiasis, penile trauma deformities, and hip stiffness due to severe rheumatoid arthritis. In women, mechanical obstacles caused by vulvar and vaginal inflammation and ulcers, tumors, or trauma can also affect normal intercourse.
●●Inappropriate sexual position can cause infertility. Inappropriate sexual position or posture can also lead to female infertility. While position is not the deciding factor, the key is for the woman to elevate her hips and lie supine for 30 minutes to 1 hour after intercourse. This helps the cervix to be immersed in the seminal pool in the posterior fornix of the vagina, facilitating sperm movement into the cervix. However, for women with a retroverted uterus, the knee-chest position makes it easier for the cervix to come into contact with the semen.
●●Inappropriate timing of intercourse can also cause infertility. Both too frequent and too infrequent sexual activity can negatively impact semen quality. Infrequent sexual activity makes it difficult for sperm to meet the released mature egg, naturally lowering the chances of conception. Human eggs are only capable of fertilization within 24 hours of ovulation, after which they rapidly age and lose their fertilization capacity. After ejaculation, sperm retain their fertilizing ability in the female reproductive tract for approximately 48 hours. Even if sperm remain in the cervical crypts, they can only survive for 3-5 days. Therefore, the timing of intercourse is crucial, especially for those with lower fertility. Some women experience light bleeding during ovulation and mistake it for menstruation, thus avoiding intercourse. This undoubtedly misses a crucial opportunity; since intercourse occurs outside of ovulation, conception is impossible.
●●Infertility caused by sexual dysfunction. This refers to the woman's inability to achieve orgasm. Influencing factors include various psychological factors, coordination and technique issues in marital sexual activity, as well as organic diseases such as diabetes and tuberculosis. During normal sexual intercourse and orgasm, the uterus and vaginal sphincter contract strongly, which helps sperm ascend. This contraction is sometimes described as creating a strong "sucking" effect, assisting sperm into the uterine cavity and migrating to the fallopian tubes for fertilization. Women with decreased libido, inability to achieve orgasm, or difficulty with intercourse lack this sperm-assisting contraction activity, thus affecting the conception process. This is one possible reason for the lower success rate of artificial insemination. However, many scholars hold a different view, arguing that uterine contractions expel rather than suck. They believe there is no necessary link between sexual dysfunction and infertility. Nevertheless, these couples may experience less frequent intercourse due to sexual dysfunction or difficulty with intercourse, thus affecting their chances of conception.
