Diagnostic and therapeutic approach for Yang deficiency constitution [Original first publication]

Having practiced Traditional Chinese Medicine (TCM) for many years, I've become quite indignant at the many people who blindly believe advertisements and immediately take "Liuwei Dihuang Wan" (a traditional Chinese medicine formula) for kidney deficiency. I'd like to share some of my experiences so you can compare them to your own body and treat accordingly. Kidney deficiency is divided into kidney yang deficiency and kidney yin deficiency, and the body is also divided into constitution-based yang deficiency and constitution-based yin deficiency. Most people are constitution-based yang deficiency; true organic yin deficiency is rare. Consider this: many people cite the following reasons for kidney deficiency: 1. Masturbation in youth; 2. Excessive sexual activity; 3. Excessive alcohol consumption; 4. Overeating. Among these, the first two are particularly common causes of impotence and premature ejaculation. TCM considers impotence and premature ejaculation to be due to "weakness of the Mingmen fire" and a weak seminal gate. You can examine yourself by touching the "chi" (尺) section of your left hand with your right hand. The cun (寸), guan (关), and chi (尺) sections of the left hand represent the heart, liver, and kidneys, respectively. If impotence or premature ejaculation is present, the pulse at the cun position (尺位) will be weak and large, disappearing upon deep pressure, or thin and wiry, barely detectable without deep pressure. Therefore, most people with this condition have a yang deficiency constitution. If they then take the yin-nourishing "Liuwei Dihuang Wan" (六味地黄丸), the consequences are predictable. So what about "deficiency of the fire of the gate of life" (命门火衰)? In traditional Chinese medicine, this refers to deficiency of the lower jiao (下焦). The lower jiao mainly refers to the spleen and kidneys. Deficiency of the primordial qi in the lower jiao and weakness of the ministerial fire (相火) lead to weak kidneys unable to exert their full power, weak spleens unable to function properly, or lower back and knee pain, or morning diarrhea—all symptoms of deficiency and cold. The ministerial fire originates in the lower jiao and circulates in the middle and upper jiao. Since the lower jiao is the place where the ministerial fire originates, it should be warmer than other parts of the body. Therefore, the lower abdomen should feel warmer to the touch than other areas, but the lower abdomen of patients with impotence and premature ejaculation is mostly cold. Furthermore, many people with impotence and premature ejaculation experience morning diarrhea, meaning they have stomach upset upon waking. The following prescription has been verified to have good effects: Codonopsis pilosula 12g, Aconitum carmichaelii 9g (added first), Dioscorea opposita 15g, Psoralea corylifolia 12g (fried and crushed), walnut kernels 9g, Cornus officinalis 12g, Poria cocos 4g, raw chicken gizzard lining 4g (crushed). Aconitum carmichaelii should be added first. After boiling, cook for 15 minutes, then add the remaining herbs and cook for another 20 minutes. Note: This prescription is for enhancing and restoring kidney function, not for aphrodisiac purposes. While a strong person can appropriately use aphrodisiacs, if a person is already weak, using aphrodisiacs is like drinking poison to quench thirst. Patients with impotence and premature ejaculation already have weakened kidney fire; it's like covering a weak fire with a shovel of wet coal—the fire will surely go out. Therefore, patients with impotence and premature ejaculation should never take aphrodisiacs.
